  • Evaluation methods and estimates of California's effectiveness in administering an intensive community-based, long-term care program for the poor, frail elderly are presented. The additional days attributable to the program that an average most-frail client spends in the community are about 20 times the additional days of an average, least-frail client. The expected additional costs over the comparison system for the most-frail client are 20 percent more than the expected additional cost for the least-frail client. Program efficiency would be advanced with improvements in client selection (increasing the percentage of most-frail clients in the program), and with the use of a staged, case management system (that is, after restoring clients to a stable situation, services are continued but the intensity of case management is reduced).
